What Is the National Diabetes Prevention Program?

The National Diabetes Prevention Program (National DPP) is a partnership of public and private organizations working to prevent or delay Type II Diabetes. This collaborative partnership between healthcare providers and community-based organizations to offer evidence-based, cost-effective interventions that have proven to help prevent Type II Diabetes.

 

The CDC-recognized lifestyle change program, that Alaunus Nutrition Services offers is a research-based online program focused to support healthy eating and physical activity for program participants.

 

Research has shown that people with pre-diabetes who take part in structured lifestyle change program and lose 5% to 7% of their body weight through healthier eating and 150 minutes of physical activity per week, can reduce their risk of developing Type II Diabetes by 58% (71% for people over 60 years old)

Who Is Eligible?

To be eligible for the referral to a CDC-recognized lifestyle changed program, participants must meet ALL the following criteria:

  • Be 18 years or older
  • Be overweight (BMI equal to or above 24, or 22 if Asian)
  • Have NOT been diagnosed with Type I or Type II Diabetes
  • NOT currently pregnant

In addition, a participant must meet one of the following criteria (unless they are enrolling in a program covered by Medicare)

  • Having had a recent blood test in the following pre-diabetes range:
    • Fasting plasma glucose - 110-125 mg/dL
    • Two-hour plasma glucose (after a 75-gm glucose load) - 140-199mg/dL 
    • A1c - 5.7-6.4%
  • Have received a result of high-risk for Type II Diabetes on the pre-diabetes risk test
  • A doctor previously diagnosed the participants of gestational diabetes

What Should National DPP Patients Expect?

The CDC-recognized DPP lifestyle change program includes 12 months of learning and support. In the first six months, participants meet weekly (online) for one hour to learn:

  • Healthy eating habits without giving up the food they love
  • How to incorporate physical activities into busy schedules
  • How to manage stress
  • Coping strategies for challenges
  • Bluetooth enabled weighing scale
  • Free app download to monitor calories and macronutrients
  • Free electronic food diary/monitor daily calorie burn

 

In the second six months participants will meet monthly for one hour to build on the skills they have learned and maintain their positive changes

What Other Benefits Can You Get, If Qualified?

  • Bi-weekly massage (feet, lymphatic drainage massage)
  • Weekly healthy food/snacks delivery (cooked/uncooked)
  • Weekly meditation sessions
  • Physical therapy sessions, if needed

Alaunus DPP Lifestyle Change Program Highlights

  •  CDC-approved curriculum with lessons, handouts, and other resources to guide you in making lifestyle modifications
  • A lifestyle coach who is specially trained to lead the program, help you learn new skills, encourage you to set and meet goals, and remain determined throughout the course of the program. The coach will also facilitate discussions that will also include fun and engaging activities.
  •  A support network with people who share the same goals and challenges as you.

Commitment

Making lifestyle changes is a long, ongoing process. The program may run for a year. During the first 6 months, there will be virtual interactive sessions. In the next 6 months, you may start meeting with your lifestyle coach and team members face to face once a month.

 

We want to ensure successful health outcomes for our participants. Thus, we encourage every participant to stay in the program for the full year to help you stick to new habits and avoid reverting into old habits.

Things You Will Learn

During the first half of the program, you will learn about healthy eating habits, physical activities, stress management, healthy food choices, and how to get back on track with your plan.

In the second half of the program, you will join sessions that will review key ideas, such as tracking diet and physical activity, setting goals, staying motivated, and overcoming challenges that hinder you from achieving your goals. Your lifestyle coach and group members will continue to support you.

Prevent or delay Type 2 Diabetes through the Alaunus Nutrition DPP Lifestyle Change Program

Alaunus DPP Lifestyle Change Program is a CDC approved Diabetic Preventive Program that features:

Your Step-by-Step Guide to DPP- Lifestyle Change Program Enrollment

              Step 1: Check your eligibility (criteria mentioned on this page), take the risk test. (https://www.cdc.gov/diabetes/takethetest/)

              Step 2: Fill in the referral form, get it signed by the physicians, email to DPP@Alaunus.us

              Step 3: Fill in the Prior Authorization Form DPP -UHC PRIOR AUTHORIZATION to submit to your insurance, once you get the approval email to      

                              DPP@ALAUNUS.US

              Step 4: Download “Healthie” app, sign up as a DPP-Client. 

              Step 5: Once your account is established, it will prompt you to a bundle of forms, fill up the forms, submit and you are all set.
